一起重大煤与瓦斯突出事故的分析

Analysis of a Serious Coal and Gas Outburst Accident

摘要: 基于行为安全模型对2006年一起重大煤与瓦斯突出事故进行研究。通过对相关人员不安全动作的分析进一步得到了事故发生的习惯原因、组织行为原因和组织文化原因，并得出组织行为错误（作业规程存在问题）是该起事故发生的主导原因。最后根据分析结果提出了加强安全文化建设、提高各级人员安全意识、训练作业人员安全作业习惯等预防措施。

Abstract: A serious coal and gas outburst accident in 2006 is studied based on the behavior safety model. The causes of the habits, the organizational behavior and the organizational culture are obtained further through analyzing unsafe acts of related persons and a conclusion is drawn that the wrong organizational behaviors (there are problems in the operation procedures) are the leading cause of the accident. Finally, the preventive measures are proposed according to the results including strengthening the safety culture construction, improving safety awareness of the staff at all levels and training the safe operation habits of employees.
